Position Overview:

We are seeking a compassionate and community-focused Registered Nurse to conduct home visits for lead testing and case management. This role is ideal for an RN who enjoys community outreach and making a direct impact on public health.

Qualifications:

• Education: Graduate of an accredited nursing program.
• Licensure: Active Registered Nurse (RN) license in the state of Kentucky. CPR certification required.
• Experience: Experience in home visitation is preferred.
• Technical Skills: Proficiency in phlebotomy (blood draws) and accurate clinical documentation.
• Soft Skills: Exceptional interpersonal, organizational, and communication skills. Comfort with community outreach is essential.

Key Responsibilities:

• Conduct in-home blood draws for lead testing.
• Provide education and follow-up case management.
• Assist with special public health projects.
• Document care accurately in patient records.
