LPN Visit Nurse Responsibilities:

• Complete initial and ongoing assessments of clients’ health status and vital signs
• Implement individualized care plans in coordination with the RN Case Manager and care team
• Administer medications and treatments per provider orders
• Monitor condition changes and promptly report updates to the interdisciplinary team
• Educate clients and families on disease management, safety, and home care best practices
• Assist with ADLs and promote comfort, safety, and independence
• Document all care accurately and thoroughly
• Communicate effectively with providers, caregivers, and team members to ensure exceptional outcomes

Licensed Practical Nurse – LPN Visit Nurse Qualifications:

• Active LPN license in Minnesota
• Minimum 1 year of home care or related clinical experience
• Strong assessment and critical thinking skills
• Excellent communication and relationship-building abilities
• Ability to work independently with confidence
• Reliable transportation and willingness to travel to patient homes
